Federal regulations require Medicaid agencies to afford individuals the opportunity to apply for Medicaid without delay. 42 CFR § 435.906. An application may be filed on the internet, by telephone, by mail, in person and through other commonly used electronic means. 42 CFR § 435.907. All applications must be signed under penalty of perjury. 42 CFR § 435.907(f). Agencies must provide assistance to individuals seeking help in a manner that is assessible to individuals with disabilities or with limited English proficiency. 42 CFR § 435.908(a). Agencies must allow individuals of the applicant’s choice to assist in the application process. 42 CFR § 435.908(b).
There are multiple ways to apply for Medicaid in Georgia. You can apply online using Georgia’s Medicaid Gateway. You can also file a paper application at any county Department of Family and Children Services (DFCS) office using Form 94, Form 297 or Form 700. We prefer Form 700. Medicaid must comply with HIPAA, so you must also sign a HIPAA release if an authorized representative will assist with your application.
